A Guide to Transfer of Stock Ownership

Instructions for transferring shares into a Custodial Account. These shares may be registered to a minor under the Uniform Transfers to Minors Act ("UTMA") or Uniform Gift to Minors Act ("UGMA").

Required Items

1. Completed Transfer of Stock Ownership Form signed by the registered owner(s)/trustee(s).

• To transfer 250 or LESS shares the form is acceptable without a Medallion Signature Guarantee.
• To transfer MORE than 250 shares the form must have a Medallion Signature Guarantee.

Each registered owner must sign his/her name exactly as it appears on the account, or the authorized person(s) must sign in his/her legal capacity.

2. The original stock certificates (if applicable).

Note: All submitted documents will be kept as part of the permanent record of transfer and will NOT be returned. Please be sure you keep a copy for your records.
