A Guide to Transfer of Stock Ownership

Instructions for transferring shares from a joint account when ALL owners are deceased and the estate has been probated.

To transfer:

50 Shares or Less

Submit items 1 through 4

 



More than 50
up to 250 Shares

Submit items 1, 2, 3, 4
and 5
            - OR -
Submit items 1, 2, 3, 4
and 6


More than 50
up to 250 Shares

Submit items 1 through 6

 





Required Items

  1. Completed Transfer of Stock Ownership Form signed by the Estate Representative of the last decedent.
  2. The original stock certificates (not required if shares are held in book-entry form).
  3. A photocopy of the death certificates of all decedents.
  4. Inheritance Tax Waiver (if applicable).
  5. A certified copy, with original signature and seal affixed, of the Certificate of Appointment of Executor(s) dated within one year of the transfer for the last decedent only.
  6. Medallion Signature Guarantee on Transfer of Stock Ownership Form.

NOTE: All submitted documents will be kept as part of the permanent record of transfer and will NOT be returned. Please be sure you keep a copy for your records.
