A Guide to Transfer of Stock Ownership

Instructions for transferring shares into a Trust Account when all the current registered owner(s) or authorized person(s) are able to sign

Required Items

1. Completed Transfer of Stock Ownership Form signed by the registered owner(s)
   /trustee(s).
   • To transfer 250 or LESS shares the form is acceptable without a Medallion Signature Guarantee.
   • To transfer MORE than 250 shares the form must have a Medallion Signature Guarantee.

   Each registered owner must sign his/her name exactly as it appears on the account, or the authorized person(s) must sign in his/her legal capacity.

2. The original stock certificates (not required if shares are held in book-entry form).
3. A photocopy of the first and last page of the trust agreement which should specify the name of the trustee(s), the date the trust was established and the complete title of the trust.

Note: All submitted documents will be kept as part of the permanent record of transfer and will NOT be returned. Please be sure you keep a copy for your records.
